Public healthcare in Madrid for newcomers
How to set up public healthcare access in Madrid, including getting your Social Security number, registering your right to care, and getting your health card.

Who is eligible for public healthcare?
Spain has an excellent public healthcare system (Sanidad Pública). However, just moving to Madrid does not automatically grant you access. You generally qualify if:
- You are working in Spain (as an employee or self-employed autónomo) and paying into Social Security.
- You are a resident pensioner or receiving state benefits.
- You are a dependent (spouse or child) of someone who meets the above criteria.
- You have an S1 form from another EU country.
(Note: If you are on a Student Visa or Non-Lucrative Visa, you are usually required to have full-coverage private health insurance instead).
Step 1: Secure a Social Security Number (NUSS)
Before you can access healthcare, you need a Social Security number. If you are hired by a company, they will usually request this for you. If you are an autónomo, you request it yourself.
- Go to the Social Security online portal (Import@ss).
- Log in using your Digital Certificate or Cl@ve.
- Select "Solicitar el número de la Seguridad Social".
- The system will instantly generate your NUSS. Download the PDF certificate.
Step 2: Register your right to healthcare (INSS)
Having a Social Security number is not enough; you must explicitly tell the government to activate your healthcare rights based on that number.
- Go to the INSS (Instituto Nacional de la Seguridad Social) portal, specifically the "Tu Seguridad Social" section.
- Log in with your Digital Certificate.
- Look for the section on "Asistencia Sanitaria" (Healthcare Assistance).
- Register yourself as the primary beneficiary. The system will issue a document certifying your "Right to Healthcare". Print this document.
Step 3: Request your Tarjeta Sanitaria (Health Card)
Now that the national government has recognized your right, you must apply for the actual physical card from the regional Madrid government (Comunidad de Madrid).
- Find your local Centro de Salud (Primary Care Clinic). You are assigned a specific clinic based on the address on your Padrón.
- Visit the clinic in person at the administration desk.
- Bring:
- Your physical ID (Passport/TIE).
- Your Padrón certificate (issued within the last 3 months).
- The INSS document proving your right to healthcare (from Step 2).
- The clerk will register you in the Madrid system, assign you a primary care doctor (médico de cabecera), and issue a temporary paper document.
- Your plastic red Tarjeta Sanitaria will arrive at your home address by mail within a few weeks.
Step 4: Booking appointments and using the system
Once you are registered in your Centro de Salud:
- Download the "Cita Sanitaria Madrid" app on your phone.
- Use it to book appointments with your assigned primary care doctor or a nurse.
- If you need a specialist (like a dermatologist or cardiologist), you must see your primary care doctor first to get a referral (volante). You cannot book specialists directly in the public system.
Common mistakes to avoid
- Going straight to the Centro de Salud: If you skip Step 2 (registering your right with the INSS), the clinic reception will turn you away because you won't appear in the national database.
- Losing your Padrón: The clinic strictly enforces addresses. If your padrón is older than 3 months, they may ask you to get a fresh one before issuing the card.
- Emergency Room abuse: For non-urgent issues, always use your Centro de Salud. Only use hospital Emergency Rooms (Urgencias) for actual emergencies, as wait times can exceed 6 hours for minor complaints.
